Katherine Ramírez
Connecticut Children’s Medical Center, Center for Care Coordination

Katherine Ramírez, has a Master’s Degree in Business Management and Organizational Leadership. She has worked as a community care coordinator for Children and Youth with Special Healthcare Needs and children who are vulnerable or at risk for poor health and developmental outcomes (birth to 21 years of age). Katherine has been with Connecticut Children’s Medical Center, Center for Care Coordination since 2009 as a Program Specialist and is knowledgeable of principles and processes in development, project management, and care coordination. Ms. Ramírez serves as the Co-Facilitator and Technical Advisor to the North Central Care Coordination Collaborative, conducts Social Network Analysis of teams, and provides consultation services to the Care Coordination Collaborative Model regionally and nationally through the Department of Public Health and Help Me Grow National Center. Katherine is a Nationally Certified Strengthening Families Protective Factors trainer.

Ms. Ramírez enjoys serving the Greater Hartford community, and is a member of United Way of Central and Northeastern Connecticut Emerging Leaders Society and Southside Institutions Neighborhood Alliance to; Connect Leaders, Cultivate Relationships, and Change Our Community. Katherine is also a Board of Directors member of the Connecticut Association of Latinos in Higher Education. She enjoys assisting organizational growth by building relationships and enhancing partnerships in the community.
